The Ordination Inquiry Packet will give you access to all the needed information about the process we use to commission, license and ordain ministers.  After you fill in the information below two things will happen. 

First, when you click the submit button a copy of the Ordination Inquiry Packet will be emailed to you in a zip file with three PDF files enclosed. 

Second, you will be re-directed to a page where you can open, read or print the Ordination Inquiry Booklet, Ordination and Credential Application and the Foundations for Ministers Enrollment form.
